Based on observations, interviews, and facility file review the licensee did not comply with the section cited above in 1 out of 4 files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff #3 scheduled TB test for 07/14/2025. Licensee to send TB test results to the LPA via email susan.deschampe@dss.ca.gov Staff #3 shall not provide care to children at the facility until passing TB test results have been received by LPA Deschampe.

Based on LPA's record review, the licensee did not comply with the section cited above in 2 out of 2 infants were missing Individual Infant Sleeping Plans in their files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviwed Safe SLeep regulations with Licensee. Licensee stated they will have the parents complete the LIC 9227 and email the copies to LPA by POC date.

Based on interview and record review, the licensee did not comply with the section cited above by not creating a 15-minute check log to record when they are physically being checked which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed Safe Sleep regulation resources with Licensee. Licensee stated they will create a sleep log for all infants in care to document their 15-minute checks. Licensee will email one week of completed nap logs for the infants in care to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above in 1 out of 1 staff member was missing a current mandated reporter certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated the staff completed the training but needs to recover the email password to obtain the copy. If the certificate cannot be retrieved, the staff member will need to complete the training and send copy of certificate to LPA by POC date.
