Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Licensee did not initially grant access to the facility.
Resolution: The licensee communicated with the helper to grant access to LPA while licensee was at her appointment. Licensee will review with staff the importance of inspection authority. Deficiency will be cleared during visit.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA observed that only one helper was present when there were 4 infant children and 2 children above 2 years old in care in the facility.
Resolution: The licensee arrived at the facility making her compliant with the ratio. Deficiency will be cleared during the visit.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee was unable to show proof/evidence of 15-minute checks were being done in the facility.
Resolution: Licensee will send verification of 15 minute checks from children under 2 years old by set due date of 10/28/2024.
