Based on records reviewed, the licensee did not comply with the above regulation, as the current facility director does not possess at least four (4) years of teaching experience as required, which poses a potential Health & Safety risk to children in care.

The needs and services plan shall be in writing and shall include the following: Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan LIC 9227...This plan shall be signed and dated by the infant’s authorized representative. This requirement was not met evidence by: LPAs were not able to review the LIC 9227 for children in care at the time of inspection. LPAs provided the director and staff a copy of the LIC 9227 and Safe Sleep handout.
.1 (f)(3) Infant Care Center Sleeping Equipment. Cribs shall be free from all loose articles and objects, including blankets and pillows. There shall be no objects hanging above or attached to the side of the crib. This requirement was not met by evidence by... Based on LPA and LPM's observation. One infant awake with a bib on and two stuffed toys in a crib, while another infant had a blanket draped over the portable crib. The staff failed to ensure all loose items were removed from inside and outside of the cribs. This poses an potential Health & Safety risk to the children in care.
