Type A citation - immediate risk to health, safety, or personal rights

Based on observation, interview, record review, the licensee did not comply with the section cited above in the facility does not have a fully qualified teacher assigned to the toddler option program who meets the educational requirements, which poses a potential health, safety or personal rights risk to persons in care.

(2) A ratio of six children to each teacher shall be maintained for all children in attendance in the toddler component. An aide who is participating in on-the-job training may be substituted for a teacher when directly supervised by a fully qualified teacher.

Based on observation, Interview, record review, by admission the licensee did not comply with the section cited above in the LPA observed teacher's assistants supervising children alone in the Toddler option program which posesd a potential health, safety or personal rights risk to persons in care.

Food Services:(a)...child care centers providing meals to children, the following shall apply: (7) Modified diets prescribed by a child's physician; (B) A child shall not be served any food to which the child's record indicates he/she has an allergy. This regulation was not met as evidenced by: Based on interview, record review, observation & by the licensee's own admission, on 10/10/2025 emergency medical services were required for a subject child due to the licensee providing a known, restricted food item during mealtime, which is an immediate risk to the health & safety of children in care.
