Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility file review, the LPA reviewed a roster that was missing children who were present.
Resolution: Licensee understands all children enrolled must be on the facility roster. Licensee updated roster during the inspection.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety & personal rights risk to the children in care. During the staff file review, the LPA was unable to review up to date Mandated Reporter certificates. The certificates on file for S1, S2, and S3 are expired.
Resolution: Licensee understands all staff working with the children must have a current Mandated Reporter certificate on file and available for review during the inspection. Mandated Reporter must renewed every two years. Training can be taken at www. Mandatedreporterca.com. Licensee agrees to submit the certificate of completion to the Department on or by POC due date of 1-23-2026.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file review the LPA reviewed sleep logs for C1, C4, C5 that were timed for every 30 minutes. All sleep logs had set times such as 12:15PM, 12:45PM, 1:15PM, etc. Licensee stated C4 and C5 lay down with the rest of the children at the same time, however, C1 does not have a sleep regular schedule due to their age.
Resolution: Licensee understands sleep logs must be conducted every 15 minutes and the times must be accurate. If a child falls asleep at 12:04PM, the next time should be 12:19PM, etc. Licensee shall submit a written statement of understanding to the Department on or by POC due date of 1-23-2026.
Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the review of staff files the LPA was unable to review a current Mandated Reporter certificate for S2. Licensee stated all staff took the training together, however, the Licensee was unable to provide proof.
Resolution: Licensee understands all staff must have a current Mandated Reporter certificate on file during the inspection. Licensee agrees to have S2 complete training and submit certificate to the Department on or by the POC due date of 8-30-24. Training can be taken at www. Mandatedreporterca .com $250 civil penalty assessed.
Based on the observation, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the facility tour the LPA observed knives in a dish rack located on the counter and knives located in an unlocked dishwasher that were accessible to the children in care, however, the children were participating in an activity away from the kitchen.
Resolution: Licensee understands all hazardous items must always be stored inaccessible from the children in care. Licensee agrees to read the above regulation and write a statement summarizing the regulation and a plan to remain in compliance. Statement is due on or by POC due date of 8-30-24. $250 civil penalty assessed.
Based on observation, the licensee did not comply with the section cited above in that there were poisons and toxins out in the open and accessible to the children in care which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility representative agreed to sbumit a letter of understanding that all toxins and poisons must be out of reach of children and key locked at all times while in facility operation. All employees and assistants to sign and date the letter of understanding. A copy of the letter much be submitted to the departmetn by the POC due date of 10/27/23.
