Based on observation, the licensee did not comply with the section cited above in that they did not have a current fire and disaster drill log for the department to review which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to conduct a fire and disaster drill and document it on a log and submit to LPA via email by the POC due date of 5/20/2025 or before.

Based on observation and interview, the licensee did not comply with the section cited above in that licensee who was present stated the other licensee was on vacation and had been away for two days at the time of inpsection which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide a letter of understanding stating they understand CCR section 102417 (a) and explaining how they will correct it and submit via email to LPA by POC due date of 5/20/25

Based on observation and record review, the licensee did not comply with the section cited above in that there were several files that did not include an acknowledgement of receipt of licensing reports (LIC 9224) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide the department a letter of understanding acknowledging they understand the section cited 1596.8595 (c) explaining how they will correct it by the POC due date of 5/20/25. Licensee is to email the LPA the letter. LICENSEE MUST HAVE ALL AUTHORIZED REPRESENTATIVES OF ENROLLED CHILDREN SIGN AND COMPLETE A LIC 9224 AND SUBMIT TO LPA BY POC DUE DATE OF 5/20/25

Based on record review, the licensee did not comply with the section cited above in that all childrens files were missing an LIC 995 which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof they have placed an LIC 995 in all children files by the POC due date of 6/21/24

Based on observation, the licensee did not comply with the section cited above in that they had several cleaning products that are poisonous in an unlocked cabinet which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to place all poisons and cleaning products in a locked cabinet by POC due date of 6/21/24

Based on record review, the licensee did not comply with the section cited above in that all children files were missing an LIC9150 which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof of an LIC 9150 for all children files by the POC due date of 6/21/24

Based on record review, the licensee did not comply with the section cited above in that licensee did not have a roster on file for the department to review which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof to the department by the POC due date of 6/21/24

Based on observation and interview, the licensee did not comply with the section cited above in that licensee stated other licensee that was not present has a full time job and not present 80 % of the time for that reason which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to remove the other licensee with an updated application and submit to LPA by POC due date of 6/21/24

Based on record review, the licensee did not comply with the section cited above in that they could not provide proof of immunization for all adults living in the home to the department posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof of immunizations for all adult by the POC due date of 6/21/24
