Based on record review, the licensee did not comply with the section cited above in that licensee did not have a current mandated reporter certificate which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to send proof of mandated reporter certificate by POC due date of 8/29/25 or sooner

Based on observation, the licensee did not comply with the section cited above in that licensee had cleaning products that stated to keep out of reach of children (cleaning wipes and toothe paste which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would email a statement stating she reviewed the regulation that covers Title 22 reg. 102417(g)(4) by the POC due date of 9/27/24

Based on record review, the licensee did not comply with the section cited above in that they did not have a sleep log for the infant incare (See LIC857) which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to send LPA 3 days worth of sleep log examples for the infant child in care by the POC due date of 9/27/24

Based on record review, the licensee did not comply with the section cited above in they did not have a current mandated reporter certificate on file to show the department which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to send proof of mandated reporter certificate for assisant by the POC due date of 9/27/24

Based on record review, the licensee did not comply with the section cited above in that they did not have an LIC 9227 for the infant child in care which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide the department proof of LIC 9227 for the infant in care by the POC due date of 9/27/24

Based on record review, the licensee did not comply with the section cited above in that they had many toxins and poisons that were not locked away which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to write a letter of understanding that they understand all toxins and poisons much be locked away in accordance with Title 22 regulations. Licensee agreed to send the letter to the department by the POC due date of 10/30/23
