SHUTE FAMILY CHILD CARE

Based on the observation, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During facility tour the LPA observed stairs that were not barricaded with two children present under the age of five. Licensee stated the gate is usually up, however, the gate was not put up during the inspection.

Resolution: Licensee understands stairs must be barricaded when children under the age of five are present. Licensee agrees to submit a written statement of understanding and will submit it to the Department on or by POC due date of 8-30-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the file review of facility records the LPA was unable to review an accurate fire drill log.

Resolution: Licensee understands a fire drill must be conducted at least once every six months and it must be documented accurately which reflects the month/day/year, length of fire drill and evacuation location. Licensee agrees to update roster and submit to the Department on or by POC due date of 8-30-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review of staff records the LPA was unable to review a current Mandated Reporter certificate for S1.

Resolution: Licensee understands all staff working with the children in care must have a current Mandated Reporter training. Licensee agrees to have S1 complete training and submit the certification of completion to the Department on or by the POC due date of 8-30-2024. Training can be completed at www. Based on the record review, the Licensee did not meet the above regulation which poses a potential Health and Safety risk to the children in care. During the file review of staff records the LPA was unable to a current CPR/1st Aid certification for S2. During the inspection there were no other staff present with CPR/1st Aid training.

Resolution: Licensee understands there must be at least one staff present with Pediatric CPR/1st Aid training through an EMSA approved vendor. Certification of completion is due on or by POC due date of 9-15-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health to the children in care. During the review of children’s files, the LPA was unable to review immunization records for C1.

Resolution: Licensee understands all children must have immunizations on file and available for review during the inspection. Licensee agrees to obtain immunization records for C1 and submit them to the Department on or by POC due date of 8-30-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the file review of facility records the LPA was unable to a current facility roster.

Resolution: Licensee understands all children must be listed on the LIC 9040 Child Care Facility Roster. Licensee agrees to update roster and submit to the Department on or by POC due date of 8-30-2024.
