Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA reviewed Mandated Reporter certificates that expired August 27 & 28, 2023. Licensee stated they do not have a current Mandated Reporter certificate available for review.
Resolution: Licensees agree to complete the Mandated Reporter Training and submit the certificate of completion to the Department on oy by the POC due date of October 27, 2023.
Based on observation, interview, record review the licensee did not comply with the section cited above. Licensee stated they did not have a completed facility roster for LPA to review, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands the importance of this regulation and will complete a facility roster and submit to LPA/Licensing on or before the stated POC due date of 09/29/2022. Corrections can be emailed to: justin.giese@dss.ca.gov
Based on observation, interview, record review the licensee did not comply with the section cited above. LPA was unable review 15 minute sleep checks for Child 1 at time of visit, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands the importance of this regulation and will conduct 15 minute sleep checks for C1 for the duration of 1 week or 5 days in care and submit to LPA/Licensing on or before the stated POC date of 09/29/2022. Corrections can be emailed to justin.giese@dss.ca.gov
Based on observation and record review, the licensee did not comply with the section cited above. Licensee was unable to show proof of completed disaster drills upon LPAs request, which poses a potential health, safety or personal rights risk to persons in care.
