Based on the observation and record review, the Licensee did not meet the above regulation which pose potential safety risk to the children in care. LPA was unable to review a current fire drill log. LPA observed Licensee counting the months from her last documented fir drill of 1-10-2025 and then writing 1-7-2026.

Resolution: Licensee understands a fire drill must be conducted at least once every six months. Licensee agrees to submit a written statement of understanding to the Department on or by POC due date of 2-19-2026.

Based on the observation, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. Upon arrival the LPA observed C1 in the playpen laying down with a pillow and toys inside.

Resolution: Licensee understands there cannot be any loose items in the playpen or crib when a child is resting/sleeping. Licensee agrees to submit a written statement of understanding to the Department on or by POC due date of 2-19-2026.

Based on the observation and record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file review the LPA was unable to review current sleep logs for C2. LPA observed Licensee filling in missing dates during the inspection and advised her not to back date the sleep logs. LPA advised to start sleep logs from today. Additionally, the logs are preset times such as 11:15AM, 11:30AM, 12:00PM…

Resolution: Licensee understands sleep logs must be completed every 15 mins every time a child under the age of 24 months takes a nap. Licensee understands time should be accurate not preset. Licensee agrees to submit a written statement of understanding to the Department on or by POC due date of 2-19-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Personal Rights risk to the children in care. During children’s file review the LPA was unable to review LIC 282 for C1, C2, C3 and C4. Licensee stated she does not have liability insurance.

Resolution: Licensee understands if she does not have Liability Insurance she must have an LIC 282 on file and available for review during inspection. Licensee agrees to obtain an LIC 282 for C1, C2, C3 and C4 and submit to the Department on 1-15-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility tour the LPA reviewed a Fire Drill Log dated 6-5-2023.

Resolution: The Licensee understands a fire drill must be conducted at least once every six months. Licensee agrees to conduct a fire drill and submit documentation to the Department on 1-15-2025.

Based on the record review, the Licensee did not meet the above regulation which poses an potential safety risk to the children in care. During children’s file review the LPA was unable to review sleep logs for C4. The Licensee stated she has the log but does not document.

Resolution: Licensee understands any child(ren) under the age of 24 months must have a sleep log on file and available for review during inspection. Licensee agrees to complete sleep logs for C4 every day until 1-29-2025 and submit to the Department on 1-30-2025. Licensee understands documentation must be completed even after 1-29-2025. Safe Sleep regulations provided.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility file review the LPA was unable to review an EMSA approved CPR/1st Aid card.

Resolution: Licensee understands CPR/1st Aid must be through an EMSA approved vendor. LPA advised Licensee to contact the local Resource & Referral for guidance. Licensee agrees to complete an EMSA approved training and submit the certificate of completion to the Department on 1-30-2025.

Based on the observation, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility file tour the LPA asked for a current Facility Roster. The Licensee was observed to be filling it out during the inspection.

Resolution: Licensee understands the Facility Roster must be current at all times and made available during inspection.

During facility file tour the LPA observed lawn care, insect killer and propane that could be accessible to the children in care. During the inspection the children were not occupying the backyard due to inclement weather (extreme wind).

Resolution: Licensee understands all items must be made inaccessible to the children in care. Licensee agrees to submit a written statement of understanding. Statement is due on 1-15-2025.
