Resolution: Licensee understands all staff providing Care & Supervision to the children in care must have a current Mandated Reporter on file. Mandated Reporter must be renewed every two years and can be completed at www. Mandatedreporterca .com. Licensee agrees to have S3 completed the training and submit the certificate on or by POC due date of 4/30/2025.

Resolution: Licensee understands all children must have an LIC 282 on file if the facility does not have liability insurance. Licensee agrees to obtain a signed LIC 282 for C1 and submit to the Department on or by POC due date of 4-30-2025.

Resolution: Licensee understands all children under the age of 24 months must have a sleep log available for review. Licensee understands all children under the age of 24 months must be observed every 15 minutes while sleep and it must be documented. Licensee agrees to submit a statement of understanding on or by POC due date of 1/16/24.

Licensee stated S2 started training but did not finish. Licensee understands training must be completed every two years. Licensee agrees to submit proof of training on or by POC due date of 1/16/24.

During the facility tour LPA observed a knife in the sink. S2 observed and agreed that the knife was accessible to the tallest child in care.

Resolution: Licensee and Assistant understand that all hazardous items including knives must be made inaccessible to children in care. Licensee agrees to submit a statement of understanding on or by close of business on 1/10/24. Licensee understands all families enrolled must sign an LIC 9224 and all families who enroll within the next 12 months must sign an LIC 9224. Place LIC 9224 in child’s file.

During facility tour LPA observed an off limits area (Laundry Room) not made inaccesble to children in care. LPA observed the door to be open.

Resolution: Licensee understands anytime there are children present all identified "off limits" area must be made inaccessible to children. under the age of five, the stairs must be barricaded. Licensee agrees to submit a statement of understanding due on or by POC due date on 1/16/24.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review an LIC 282 for six of nine children present at the facility.

Resolution: Licensee understands if she does not have Liability Insurance she must have an LIC 282 on file. Missing LIC 282s for all children are due on or by POC due date of 1/16/24.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review current CPR/1st Aid certificates for herself and S2.

Resolution: Licensee understands at least one staff present must have completed the CPR/1st Aid training and the certification of completion must be made available for review. Licensee stated her training was completed however she does not have the card. Licensee agrees to submit proof of training on or by POC due date of 1/23/24.

Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. Upon arrival LPA observed S2 alone with nine children. S2 stated the Licensee does school drop offs in the morning and was not present at the facility. Licensee arrived at the facility 10-15 minutes after the inspection started.

Resolution: Licensee understands as a large family child care facility if she or an Assistant is alone she must operate as a small family child care with a max of eight children. Licensee agrees to submit a statement of understanding and how she plans to stay in compliance when she is away from the facility on or by 1/10/24. Licensee understands all families enrolled must sign an LIC 9224 and all families who enroll within the next 12 months must sign an LIC 9224. Place LIC 9224 in child's file.
