Based on observation and record review, the licensee did not comply with the section cited above in that the licensee could not provide proof of mandated reporter cert. for herself which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to email LPA her updated mandated reporter certificate via email by the POC due date of 1/17/2025 to claudia.caywood@dss.ca.gov

Resolution: Licensee agreed to provide proof of current Measles immunization by the POC due date of 1/17/2025 to claudia.caywood@dss.ca.gov
Based on record review, the licensee did not comply with the section cited above in childrens files did not have an LIC 282 for the department to review which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to email LPA LIC282 for all children files.
