Based on observation, the licensee did not comply with the section cited above in the LPA observed batteries, matches, medicines & supplements, houshold cleaners, insect killers, bleach and other toxic and poisonous household items placed in a lower kitchen drawer with a broken latch and kitchen cabinets without lockswhich poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children and submit proof that items are made inaccessible to children, by photo and/or video.

Based on observation, the licensee did not comply with the section cited above where a child under 2 years of age was observed napping in a play yard with a pillow and blanket, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure Cribs or play yards shall be free from all loose articles and objects and agrees to submit a written statement understanding and a plan to ensure the regulation is met at all times. The licensee also agrees to submit the written statement, signed and dated to the department by fax, mail or e-mail.

Based on record review, the licensee did not comply with the section cited above in 2 children in care are misssing proof of immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees ensure each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled. The licensee also agrees to submit proof of immunizations for 2 subject children by fax, mail or e-mail.

Licensee understands Mandated Reporter training must be completed every two years and can be take at www. manadatedreporterca.com Licensee agrees to submit completion of training certificates to the Department on or by POC due date of 2-29-2024.

Based on the record review, the Licensee did not meet the above regulation poses a potential Health, Safety & Personal Rights risk to the children in care. During review of staff records LPA was unable to review required regulations for S2.

Resolution: Licensee understands all individuals who require immunizations must have them available for review during inspection. Licensee agrees to have S2 submit proof of immunization of MMR, DTap and TB or a Titers Test, then Licensee will submit them to the Department on or by POC due date of 2-29-2024.

Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During facility tour LPA observed several hazardous items that were accessible to the children in care which included cleaning solutions in the restroom, three propane tanks in the backyard, vitamins/supplements in the kitchen, dog poop and both side yards had a lot of stacked and unused furniture and tools.

Resolution: Licensee understands all hazardous items must be made inaccessible to children in care. Licensee agrees to submit a written statement stating how she will remain in compliance with the regulation and provide a hazard free environment for the children in care. Licensee agrees to submit pictures of hazardous items removed as discussed during tour. Statement and pictures are due on 2-14-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review an LIC 995A for C1, C2, C3, C4, C5, and C6.

Resolution: Licensee all required forms must be on file and available for review during inspection which includes Family Child Care Home Notification of Parents’ Rights, LIC 995A. Licensee agrees to obtain an LIC 995A for C1, C2, C3, C4, C5, and C6 then submit to the Department on orby POC due date of 2-29-2024. LPA provided an LIC 995A during inspection.

Resolution: Licensee understands that all children under the age of 12 months must have an LIC 9227 on file and available for review during inspection. Licensee agrees to obtain an LIC 9227 for C1 and submit to the Department on or by POC due date of 2-29-2024. LPA provided an LIC 9227 during inspection.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review immunization records for C2, C4 and C5.

Resolution: Licensee understands all children must have immunizations on file and available for review during inspection. Additionally, Licensee understands all immunizations must be transferred to the CDPH 286. Licensee agrees to obtain immunization records for C2, C4 and C5 then submit them to the Department on or by POC due date of 2-29-2024. LPA provided a CDPH 286 during inspection.

Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During facility tour LPA observed an infant in a Fisher Price Rocker on top of the kitchen counter. When asked to move the child, the Licensee left the child in the rocker and put the entire rocker with the child in a bassinet. LPA reviewed the warning label which states, “NEVER lift or carry child in rocker” “ALWAYS

Resolution: place rocker on floor. Never use on any elevated surface.” “NEVER use on a bed, sofa, cushion, or other soft surface." Licensee understands baby equipment must be used for its intended purpose and must be used according to the manufacture’s labels and warning labels. Licensee agrees to submit a written statement stating how she will remain in compliance with the regulation and provide a safe environment for the children in care. Statement is due on 2-14-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review sleep logs for C1, C2, and C3.

Resolution: Licensee understands all children under the age of 24 months must have a sleep log available for review. Sleep logs must include the child’s name, date and times that reflect a 15 minute check every time the child is sleep. Licensee agrees to submit sleep logs from 2-14-2024 through 2-28-2024 and submit to the Department on the POC due date of 2-29-2024. LPA provided a copy of Safe Sleep regulations during inspection.
