Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the file review the LPA was unable to review a current Mandated Reporter certificate for S1 or S3.
Resolution: Licensee understands all staff must have a current Mandated Reporter on file. Licensee understands Mandated Reporter training must be taken every two years. Licensee agrees to complete training and have S3 take training, then submit both certificates of completion to the Department on or by POC due date of 7-15-2024.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the file review the LPA was unable to review a current facility roster.
Resolution: Licensee understands all staff must have a current roster on file with all children enrolled. Licensee agrees to update the roster and submit to the Department on or by POC due date of 7-15-2024.
Based on the file review the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the file review LPA was unable to review current sleep logs for C3. Additionally, the sleep log times are set times (i.e 12:00PM, 12:15PM, 12:30PM, 3:30PM, 3:45PM,). Licensee understands the times must reflect the actual time the child goes to sleeps.
Resolution: Licensee understands sleep logs must be current and on file. Licensee agrees to read and submit a statement of understanding to the Department on or by POC due date. This is a repeat citation. $250 is assessed.
Based on the observation the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the facility tour LPA observed C4 being placed in the playpen upon arrival. As the tour continued LPA observed C4 awake with a blanket over him/her.
Resolution: Licensee agrees to read the regulations and write a statement of understanding to the Department on or by POC date. Licensee understands loose articles and objects cannot be placed in the play yards. Safe Sleep regulations were provided.
Based on the file review the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review LPA was unable to review MMR and TB records for S2. Licensee stated there is no records available for review for S2.
Resolution: Licensee agrees to have S2 provide immunization records for MMR & TB and submit to the Department on or by POC due date. LPA explained if S2 already received the MMR vaccine and does not have records then a Titers Test can be taken.
Based on the record review the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review the LPA was unable to review an LIC 9224 for C4.
Resolution: Licensee agrees to obtain an LIC 9224 for C4 from Authorized Representative and submit to the Department on or by POC due date. Licensee understands all children under the age of 12 months must have an LIC 9224 on file. Safe Sleep regulations provided during inspection.
Based on the file review the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review LPA was unable to review sleep logs for C3 and C4. Licensee stated she thought the regulation was for children under one year old. Licensee understands all Safe Sleep regulations apply to children under the age of 24 months.
Resolution: Licensee agrees to read and submit a statement of understanding to the Department on or by POC due date.
