KURERA FAMILY CHILD CARE

Based on the observation the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility tour LPA observed off limits areas not being made inaccessible to children in care. Off limit areas have latches at the top of the doors, however, the latches were not latched. Additionally, the LPA observed one of the doors open.

Resolution: Licensee understands anytime there are children present all identified "off limits" area must be made inaccessible to children. Licensee agrees to submit a written statement of understanding on or by the POC due date of 10-15-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the review of children’s records the LPA was unable to review LIC 9150 Parent Notification Additional Children in Care for C1.

Resolution: Licensee understands all children must have all required licensing forms prior to the child attending. Licensee understands all required forms must be available for review during the inspection. Licensee agrees to obtain the Parent Notification Additional Children for then submit to the Department on or by POC due date of 10-15-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the review of children’s records the LPA was unable to review LIC 995A Notification of Parents Rights for C1 and C3.

Resolution: Licensee understands all children must have all required licensing forms prior to the child attending. Licensee understands all required forms must be available for review during the inspection. Licensee agrees to obtain the Notification of Parents Rights for C1 and C3 then submit to the Department on or by POC due date of 10-15-2024.

Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the inspection the LPA was unable to review sleep logs for C3. Licensee stated she was informed that the sleep logs were to be completed until the child turned one.

Resolution: LPA informed the Licensee that sleep logs must be completed until the child turns two. Licensee agrees to submit a written statement of understanding on the regulation. Safe Sleep regulations were provided during the inspection.
