KIRKWOOD FAMILY CHILD CARE

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility tour the LPA reviewed a fire drill log which shows the last documented fire drill on 2-27-2025.

Resolution: Licensee understands a fire drill must be conducted at least once every six months. Licensee agrees to conduct a fire drill and submit documentation to the Department on or by POC due date of 1-25-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the staff file review the LPA was unable to review a valid CPR/1st Aid certificate for S1 or S2.

Resolution: Licensee understands there shall be at least one staff always present at the facility with a valid CPR/1st Aid. The valid CPR/1st Aid certificate should be available for review during the inspection. Licensee agrees to have S1 and/or S2 complete CPR/1st Aid training and submit the certificate of completion to the Department on or by POC due date of 1-23-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunizations for C3.

Resolution: Licensee understands all children enrolled must have immunizations on file and available for review during inspection. Licensee agrees to obtain immunization records and submit them to the Department on or by POC due date of 1-15-2026.

Based on the interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the interview with the Licensee she disclosed her or her Assistant do not have an updated Mandated Reporter certificate.

Resolution: Licensee agrees to complete Mandated Reporter training. Licensee also agrees to have her Assistant complete Mandated Reporter training. Both certificates are due on or by POC due date of 11-27-2023. Training can be completed at www.mandatedreporterca.com

Based on the record review, the Licensee did not meet the above regulation which poses an a potential Health, Safety & Personal Rights risk to the children in care. During record review, LPA reviewed a CPR/1st Aid card for Licensee and Assistant that is not an EMSA approved vendor.

Resolution: Licensee agrees to have her and her Assistant complete CPR/1st Aid training with an EMSA approved vendor and submit a current certificate to the Department on or by the POC date of 11-27-2023.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the review of children’s file LPA was unable to review an LIC 9150 and LIC 627 for C3 and C5.

Resolution: Licensee agrees to obtain signed LIC 627 & LIC 9150 for C3 and C5. Forms are due on or by POC due date of 11/27/2023.
