Based on the observation, record review, and interview the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Upon arrival the LPA was greeted by Jennette who stated she is the Director and has been the Director for six years. Jennette is not the Director on file with the Department and the Department has not been notified of the change. Director stated the facility has an AM and PM Director.

Based on the record review the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During children’s file reviews the LPA was unable to review an LIC 701 Physician’s Report for C1, C2 or C3.

Based on observation, interview and record review, the licensee did not comply with the section cited above in the licensee was unable to provide proof of the required Preventative Health and Safety training, including childhood nutrition and lead components for the director or opening and closing staff designated to substitute in the director's absence, directorwhich poses a potential health, safety or personal rights risk to persons in care.
