Based on the observation, record review, and interview the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Upon arrival the LPA was greeted by Jennette who stated she is the Director and has been the Director for six years. Jennette is not the Director on file with the Department and the Department has not been notified of the change. Director stated the facility has an AM and PM Director.
