Based on observation, record review, the licensee did not comply with the section in that neither the licensee or assistant did not have a current mandated reporter certificate on file for the department to review which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to provide the department a copy of a current mandated reporter certificate by the POC due date of 2/9/24
Based on observation and record review, the licensee did not comply with the section cited above in that they could not provide current immunizations for licensee and assistants listed on the LIC 859 which posed a potential health, safety or personal rights risk to persons in care.
