Based on observation and interview , the licensee did not comply with the section cited above in that there were cleaning compounds on the bathroom counter top and under the bathroom sink during the inspection and while children 4 were in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide a statement of acknowledgement stating they understand the Title 22 Regulation section 102417(g)(4) and submit to the department by POC due date of 9/27/2024. Licensee removed cleaning compounds to a locked location during the inspection.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in that they did not have proof of immunization for MMR and Tdap which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof of immunization from MMR and Tdap by the POC due date of 10/28/24

Based on [(observation) the licensee did not comply with the section cited above in that LPA observed facility out of ratio with 3 infants and 5 preschool present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit written of understaning of ratio as a large and when assistant is not present for operation as a small by POC due date 11/08/23. LPA provided hardcopy of ratio and capacity sheet to Licensee.

Based on [(observation) , the licensee did not comply with the section cited above in that the facility has cleaners and disinfectant spray accessible on bathroom shelf/ under cabinet and sharps/batteries accessible in kitchen- which poses/posed a potential health, safety or personal rights risk to persons in care

Resolution: Facility will submit proof of removal of cleaners and sharps from the bathroom and kitchen and made inaccessible to children in care by POC due date- 11/09/23 or earlier.

Based on (record review)], the licensee did not comply with the section cited above in that last drill was conducted in April 2023 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will send proof of fire drill completed (video) and picture of updated log to the department by POC due date 11/10/23 or earlier.

Based on (interview) and (record review)], the licensee did not comply with the section cited above in that 15 minute documentation is not completed as required for safe sleep for all infants enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit 15 minute safe sleep logs for all infants enrolled to the department by POC due date 11/10/23.
