Based on interview and record review, the licensee did not comply with the section cited above in that they could not proivde vaccination records for two infant children in care which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licenee stated she will ask the parent to provide her a medical exception letter written by the childrens physician stating they are delaying their vaccination by a certain date. The POC due date is 9/30/24 and must be emailed to LPA.
