Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During facility record review the LPA reviewed a fire drill log documenting the last drill was conducted on 8-20-2024. The Licensee understands fire drills must be conducted at least once every six months.

Resolution: Licensee agrees to conduct a fire drill and send documentation to the Department on or by POC due date of 3-14-2025.

Based on the staff record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During staff record review the LPA reviewed expired Mandated Reporter certificates for S1 and S1. Licensee understands all adults working with the children must have a current Mandated Reporter on file. Licensee understands Mandated Reporter training must be conducted every two years. Based on the children record review, the Licensee did not meet the above regulation which poses a potential Safety & Health risk to the children in care. During children’s record review the LPA was unable to review sleep logs for C1. The Licensee stated she believed the sleep logs were for children up to one year old. Licensee understands sleep logs must be completed for children up to two years old.

Resolution: Licensee agrees to complete sleep logs from today through 3-14-2025 and submit all records by close of business on 3-14-2025. Licensee agrees to continue to complete sleep logs for all children under two.

Based on the review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA reviewed a CPR/1st Aid for S1 and S2 that was not EMSA approved.

Resolution: Licensee understands CPR/1st Aid training must be EMSA approved. Licensee agrees to enroll & complete training and submit proof of completion to the Department on or by POC due date of 1-25-24.

Based on the review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA reviewed immunizations for S1 which was missing MMR.

Resolution: Licensee understands all staff must have proof of DTap/MMR/TB. Licensee agrees to submit proof of MMR to the Department on or by POC due date of 1-25-24.
