Based on record review, the licensee did not comply with the section cited above in 3 out of 11 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide completed copies of Consent for Medical Attention LIC627) for Child #1, Child #2, and Child #3. The plan of correction shall be submitted to CCLD on or before 12/31/2025.

Based on record review, the licensee did not comply with the section cited above in 3 out of 11 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide completed copies of Notification of Parents Rights (LIC995A) for Child #1, Child #2, and Child #3. The plan of correction shall be submitted to CCLD on or before 12/31/2025.

Based on an interview with the licensee, the licensee did not comply with the section cited above in 1 out of 1 infant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide 2 weeks of the required 15 minute infant sleep documentation for Infant #4. The plan of correction shall be submitted to CCLD on or before 12/31/2025.

Based on record review, the licensee did not comply with the section cited above in 3 out of 11 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide completed copies of the Parent Notification, Additional Children in Care (LIC9150) for Child #1, Child #2, and Child #3. The plan of correction shall be submitted to CCLD on or before 12/31/2025.

Based on LPA's observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide a written statement on how she will comply with the capacity requirements for a Small Family Child Care Home when an assistant is not present. The plan of correction shall be submitted to CCLD on or before 12/8/2025.

Based on record review, the licensee did not comply with the section cited above in 4 out of 11 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide completed copies of the Affidavit Regarding Liability Insurance (LIC282) for Child #1, Child #2, Child #3, and Child #4, to CCLD on or before 12/31/2025.

Based on observation and record review the licensee did not comply with the section cited above in which adult residing in the home does not have a current livescan which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee must have adult residing in the home to have a current livescan and submit proof that livescan was completed and have all families complete a LIC 9224 and submit to CCLD by 12/3/24.

Based on observation, the licensee did not comply with the section cited above in which LPA observed 1 child over the age of 12 months sleeping in the pack n play with blankets which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will review the safe sleep regulations and submit a statement to CCLD stating that they understand the safe sleep regulations

Based on record review, the licensee did not comply with the section cited above in licensee did not have a current fire drill log updated which poses a potential health, safety or personal rights risk to persons in care.
