BOTT, DENA FAMILY CHILD CARE HOME

Based on observation, the licensee did not comply with the section cited above due to large cobwebs throughout the home, dust covered furnace filter, fan over lunch table had a thick cover of dust, popcorn ceiling debris was on the fireplace, debris on floors, trash can had food particles on it, carpets contained dirt and stains, inside of refrigerator contained food particles and home had foul odor which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will prepapre a detailed plan of how she will clean her entire home and send a copy to LPA Cunningham at nicolette.cunningham@dss.ca.gov.

Based on observation and interview, the licensee did not comply with the section cited above in multiple broken toys, broken glider chair, and broken fixtures throughout the home which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will remove broken toys and fixtures and send pictures to LPA Cunningham at nicolette.cunningham@dss.ca.gov.

Based on observation and interview, the licensee did not comply with the section cited above in multiple large pieces of furniture were not secured, several items were stacked up high and a power strip was plugged in on the front porch with outlets uncovered which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will clear items that are stacked up high, hire someone to come in and secure furniture, remove power strip and send pictures to LPA Cunningham at nicolette.cunningham@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above due to the home had a foul odor, the furnace filter was extremely dusty, the fan above table where children eat had a layer of dust and small toys, papers, stickers, toothpick and debris were throughout the home which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will prepare a plan to ensure the home is enjoyable and comfortable for all infants and children and send a copy of the plan to LPA Cunningham at nicolette.cunningham@dss.ca.gov.
