NOT SUBMITTING DOCS WITHIN 2 WKS OF SAMPLING DATE (a) Written Directives Post -Testing Requirements and Information (a) A fully completed, and signed as applicable, copy of the following documents shall be sent by the Child Care Center licensee to the Licensing Program Analyst (LPA) within 2 weeks of the completed sampling date: (1) External Water Sampler Self-Certification Form (LIC 9275) for certified external water samplers. (2) Facility Sketch (LIC 999) fully labeled with the locations of all water outlets, including outlets that will not be sampled. (3) Child Care Center Sampling Checklist Form (LIC 9276) This requirement was not met by: facility did not submit required documentation within required time.
Based on records reviewed and interviews conducted, it was determined that the preschool is not a Spanish immersion school as the Parent handbook describes. This poses a potential risk to the health, safety, and personal rights of children in care.

LPA reviewed a sample of staff files and observed files were complete with documentation of meeting educational qualification requirements; however, some staff files were missing immunization records, and other required employment records. Director Sanchez was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed.
