Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care. THIS REQUIREMENT WAS NOT MET AS EVIDENCED BY A REVIEW OF RECORDS WHICH REVEALED THAT THE LICENSEES MANDATED REPORTER TRAINING CERTIFICATES ARE EXPIRED

Resolution: Licensees will update their mandated reporter training and submit copies of the updated certificates by 5/9/2025

A REVIEW OF RECORDS REVEALED SEVERAL CHILDREN DO NOT HAVE IMMUNIZATION RECORDS IN FILE. SOME HAVE IMMUNIZATION BLUE CARDS BUT NO IMMUNIZATION RECORDS TO COMPARE.

Resolution: LICENSEE WILL HAVE EACH CHILD'S PARENT/GUARDIAN SUBMIT THEIR CHILD'S IMMUNIZATION RECORDS. LICENSEE WILL SUBMIT COPIES OF THE RECORDS TO COMMUNITY CARE LICENSING BY 6/23/23

TODAY UPON ARRIVAL THE FCCH IS OVER CAPACITY BY 2 CHILDREN. LICENSEE SAYS 2 OF THE CHILDREN ARE "HELPERS". BOTH MINORS ARE UNDER 14 YEARS OLD.

Resolution: LICENSEE MUST REDUCE HER CAPACITY BY 2 CHILDREN. LICENSEE CAN CARE FOR UP TO 14 CHILDREN AT ONE TIME. WHEN AT FULL CAPACITY, 2 OF THE CHILDREN MUST BE SCHOOL AGE AND ENROLLED/ATTENDING ELEMENTARY SCHOOL.
