Based on observation, the licensee did not comply with the section cited above: LPA observed a barbecue grill made of loose cement blocks that in the outdoor play area accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will remove the barbecue grill from the accessible outdoor play area. LIcensee will send proof of removal to LPA via picture by POC due date.
Based on record review, the licensee did not comply with the section cited above: LPA did not observe Licensee's immunization records for pertussis, measles, and tuberculosis which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will proof of immunizations to LPA via email by POC due date.
