Based on observation, the licensee did not comply with the section cited above facility does not have currently serviced fire extinguisher which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure the maintain a new or serviced fire extinguisher annually. Licensee will service fire extinguisher and submit proof to LPA by POC Due Date via email.
Based on record review, the licensee did not comply with the section cited above in Licensee and staff 1 (S1) was missing proof of Mandated Reporter Training Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure all employees maintain a valid Mandated Training Certificate on file. Licnesee will submit proof of Mandated Reporter Training Certificate for both Licensee and S1 by POC Due Date via email.
Based on observation, the licensee did not comply with the section cited above in that thorny plants were observed in outdoor play area which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Facilty Representative, the thorny plants will be removed and a photo will be sent to LPA by POC due date. Until then the children will not play outside.
