Menus are posted on the parent board upon entrance of the facility once a month. In the caterpillar classroom (2-3 years old) LPAs observed cleaning supplies stored in a high cabinet out of reach of children. LPAs observed a filtered water container and cups available for children. Children in this classroom are offered breakfast, lunch and a snack by the facility. There is a restroom in the classroom which contained one sink and one toilet. Per Facility Representative, the infant program uses the preschool yard for outdoor play. Per Facility Representative, the infant program has been using this yard for a few years. There is a yard at the front of the facility which was previously used by the infants but per facility representative, is not being used due to it not being approved for use by Community Care Licensing Division (CCLD). LPAs informed Director that a waiver or prior approval from CCLD is required any time there is a change in the physical plant for a facility- A type B citation will be issued. During the inspection LPAs observed preschool children entering the Tiny Tots classroom to access the play yard. Per Director, that is the only way to access play yard. LPAs informed Director that each licensed component shall be physically separate from each other. A type B citation will be issued. LPAs reviewed children's records. Two out of three children's records were missing the physician report and the individual feeding plan. All three files reviewed did not have the 15 minute sleep log. LPAs reviewed staff files. Staff files did not contain the child care provider training certificate. At least one staff

Based on interview the facility did not follow reporting requirments per Title 22. staff were notified by the parent on two separate occasions regarding marks on C1 by parent. Staff did not provide C1’s parent to Community Care Licensing which poses a potential health and safety risk to children in care.
