MURRAY FAMILY CHILD CARE

Based on observations the licensee did not comply with the section cited above in LPA observed multiple disinfectants sprays and container of Comet cleaner stored underneath the kitchen, observed personal hygiene products and Listerine mouthwash stored in a cabinet above the toilet unsecured, and LPA observed accessible Insect Killer spray located on the barbeque pit which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure all poisons are locked. Licnesee will ensure all cleaning products and personal hygiene products are inaccessible to children in care. Licensee will keep posions locked in garage. Licnesee will add a key lock for cleaning products and hygiene products in restroom. Licensee will provide proof by POC Due Date via email.

Based on record review, the licensee did not comply with the section cited above in LPA observed a fire extinguisher located in kitchen room with no service tag or no proof of purchase which poses a potential health, safety or personal rights risk to persons in care

Resolution: Licensee will ensure the maintain a new or serviced fire extinguisher annually. Licensee will purchase fire extinguisher and submit proof to LPA by POC Due Date via email.

Based on record review, the licensee did not comply with the section cited above Licensee was missing a current Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will ensure she has a current Mandated Reporter Training Certificate. Licensee will provided proof via email of a current Mandated Reporter Training Certificate.

Based on record review, the licensee did not comply with the section cited above in Child 1 (C1) is missing proof of LIC 282 and C2 is missing proof of LIC 282, LIC 627, and LIC 995A which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure all children enrolled have a file readily available. Licensee will provide proof of child 1 (C1) missing document LIC 282 and child 2 (C2) missing documents LIC 282, LIC 627, and LIC 995A by POC Due Date via email.

Based on record review, the licensee did not comply with the section cited above: Licensee did not have infant sleep log for 2 out of 2 infants] which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of infant sleep log to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above: Licensee did not have LIC 995A available for review for 4 out of 4 children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of completed LIC 995A to LPA via email by POC due date.

Based on observation, the licensee did not comply with the section cited above: LPA observed fire extinguisher with no proof of purchase or service tag within 12 months.which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee indicated she will purchase a new extinguisher or get it serviced.

Based on record review, the licensee did not comply with the section cited above: Licensee does not have evidence of current/renewed CPR which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of renewed CPR to the department.

Based on record review, the licensee did not comply with the section cited above: Licensee does not have completed Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is in processs of renewing and will provide proof of renewal to CDSS.
