Based on observation LPA observed poisons located in an unlocked shed in backyard. Cleaning products are kept in unlocked downstairs closet. LPA observed in garage in unlocked cabinet above washer and dryer laundry detergent. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will esnure all poison and cleaning products are centrally stored and inaccessible to children in care. Licensee will review the labeling on products and lock any products that state keep out of reach of children. Licensee will provide images of poisons and cleaning products removed from down stairs closet and cabinent above washer and dryer. Licensee will provide proof shed is securely locked by POC Due Date.
Based on observation, the licensee did not comply with the section cited above in the facility did not contain a fire extinguisher with proof of purchase or service tag within the year which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure to have fire extinguisher 2A10BC with either proof of purchase or service tag within the year. Licensee will provide proof of fire extinguisher 2A10BC and supporting documentation via email by POC Due Date.

Resolution: Licensee will ensure stairs shall be fenced or barricaded. Licensee will submit proof of stairs enclosed by POC Due Date via emial.
Based on observation, the licensee did not comply with the section cited above: Licensee did not have a fire extinguisher purchased or serviced within the last year which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of a newly purchased or serviced fire extinguisher to LPA via email by POC due date.
