Based on observation, the licensee did not comply with the section cited above in an electrical fireplace in living room unsecured and accessible to children. The electrical fireplace has small heating vents which poses a potential health and safety to children in carewhich poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure fireplaces shall be screened to prevent access to children. Licensee Jasmine states will remove electrical fire place from facility. Licensee will provide proof of removal to LPA Reyes by POC Due Date via email.
Based on interview and record review, the licensee did not comply with the section cited above in C1's file was not not available for review. Per Licensee Jasmine C1’s file is with their guardian which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Jasmine will ensure each child has a file readily available for review. Licensee Jasmine will provide proof by POC Due Date C1's LIC 282, Immunization Record, LIC 700, LIC 627, LIC 995A, and LIC 9227 via email to LPA Reyes.
Based on observation, the licensee did not comply with the section cited above due to the child #1 was laying in a play yard with sheet not fitter or appropriate to the mattress size which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Assistant, the sheet will be removed and replaced with one that fits. A photo will be sent to LPA by POC due date
Based on observation, the licensee did not comply with the section cited above in 1 out of 1 infant (age 7months) was napping in play yard wearing an oversized sleep sac size 12-24 months (photo taken) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per phone conversation with licensee Jasmine Martinez, they don't agree with defeciency and will appeal
