Based on observation, at 9:50 AM LPA observed the license posted inside the classroom (1010) known as K1 used by the preschool. At 10:56 AM LPA observed children are picked up and dropped off at the gate located in the Elementary School next to room 1150. the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Based on interview and record review, the licensee did not comply with having all staff files readily available which poses a potential health, safety or personal rights risk to persons in care.
Based on observation at 10:13 AM LPA observed the play structure located in the right side of the playground was missing a plastic cover in one of the climbing area, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Based on observation at 10:16 AM LPA observed exposed pipes along the wall where children play, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Based on observation at 10:11 AM LPA observed the cushioning around the climbing equipment in the play area located in the left side of the playground is cracked, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
