Based on record review, the licensee did not comply with the section cited above in staff 1 did not have a valid Pediatric CPR and First Aid approved by Emergecny Medical Services Authority EMSA which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure required employees have a current and valid Pediatric CPR/First Aid. Licensee will provide proof of a valid Pediatric CPR/First Aid approved by Emergency Medical Services Authority EMSA for staff 1 . POC will be cleared by visit.
Based on record review, the licensee did not comply with the section cited above in volunteer 1 (V1) was missing proof of immunization of measles, pertussis, influenza, and TB which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licnesee will ensure all employees have proof of immunization record. Licnesee will provide proof of immunization measles, pertussis, influenza, and TB for V1. Plan of correction will be cleared by visit.
Based on record review, the licensee did not comply with ensuring all children in care had immunizations in that 1 out of 5 children's files were missing immunizations which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain immunizations from parent and submit photographic proof via email to LPA
Based on observation the licensee did not comply with ensuring child's saftey in that child was strapped into a moving swing placed on top of a table, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed child from swing and placed swing in an off limit area. Per Licensee she will send swing home with child.
