LAVELLE FAMILY CHILD CARE

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed a fire extinguisher in the kitchen which pressure gauge indicated that the extinguisher needs to be recharged and with a purchase receipt of 5/17/23 which poses a potential health, safety or personal rights risk to persons in care

Resolution: Licensee will ensure the maintain a new or serviced fire extinguisher annually. Licensee will service fire extinguisher or purchase a fire extinguisher. POC will be cleared by a visit.

Based on interview, the licensee did not comply with the section cited above in LPA was unable to review children's files per licensee and facility representative files are key locked in a cabinet and facility repersentative did not have access which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure all children files are readily available. POC will be cleared by visit.

Based on interview, the licensee did not comply with the section cited above in LPA was unable to review staff files per licensee and facility representative files are key locked in a cabinet and facility repersentative did not have access which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure all staff files are readily available. POC will be cleared by visit.

Based on record review, the licensee did not comply with the section cited above at 11:37AM on 8/6/25 a check of the Department of Social Services Guardian Background Check System indicates Adult 1 (A1) clearance Determination is in process which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure all adults residing in the home are cleared and associated to faciliy. Licensee will ensure Adult 1 (A1) is cleared and associated if going to reside in the home. A1 is not to have direct contact with the children enrolled until cleared and associated. POC will be cleared by visit.

Based on interview, the licensee did not comply with the section cited above, licensee confirmed that the immunization records for 3 out of 3 adults providing supervision to children in care are not available which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Proof of correction will be sent to LPA vial email by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 3 adults present supervising children have proof of Pediatric CPR/1st Aid training which posesa potential health, safety or personal rights risk to persons in care.

Resolution: Proof of correction will be submitted to the department by email by POC due date

Based on licensee confirmation the licensee did not comply with the section cited above in that the children's roster was not available which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Proof of correction will be sent to LPA via email by POC due date (5/5/23)

Based on interview, the licensee did not comply with the section cited above in 9 out of 9 children in care did not have faciliy files including proof of immuniczations, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Proof of correction will be sent to LPA via email by POC due date.

Based on observation and interview, the licensee did not comply with the section cited above in that licensee confirmed 3 out of 3 adults present supervising children do not have facility files which posesa potential health, safety or personal rights risk to persons in care.

Resolution: Proof of correction will be submitted to the department by email by POC due date

Based on observation, the licensee did not comply with the section cited above in that there were power tools, paint, missing outlet wall plates and missing switch cover in bedroom that children use which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the power tools and made the room inaccessible to children in care. The outlet and light swith will be covered. Proof of corrections will be sent to LPA by POC due date (within 24 hours).

Based on observation, the licensee did not comply with the section cited above due to a flooring contractor was present during daycare hours making alterations to the bathroom located in bedroom used by children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete and provide to the depatment a completed LIC 624B reporting the alterations to the home and sent to LPA via email by POC due date
