Based on observation and record review, the licensee did not comply with the section cited above in LPA observed a fire extinguisher in the kitchen which pressure gauge indicated that the extinguisher needs to be recharged and with a purchase receipt of 5/17/23 which poses a potential health, safety or personal rights risk to persons in care

Resolution: Licensee will ensure the maintain a new or serviced fire extinguisher annually. Licensee will service fire extinguisher or purchase a fire extinguisher. POC will be cleared by a visit.

Resolution: Licensee will ensure all adults residing in the home are cleared and associated to faciliy. Licensee will ensure Adult 1 (A1) is cleared and associated if going to reside in the home. A1 is not to have direct contact with the children enrolled until cleared and associated. POC will be cleared by visit.

Based on observation, the licensee did not comply with the section cited above in that there were power tools, paint, missing outlet wall plates and missing switch cover in bedroom that children use which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the power tools and made the room inaccessible to children in care. The outlet and light swith will be covered. Proof of corrections will be sent to LPA by POC due date (within 24 hours).

Based on observation, the licensee did not comply with the section cited above due to a flooring contractor was present during daycare hours making alterations to the bathroom located in bedroom used by children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete and provide to the depatment a completed LIC 624B reporting the alterations to the home and sent to LPA via email by POC due date
