Based on observation, interview and record review , the licensee did not comply with the section cited above in one staff missing complete immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to obtain completed staff immunization records and submit picture proof via email.

Based on observation and interview, the licensee did not comply with the section cited above in outdoor play structure not securely anchored to ground which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to anchor play structure to ground and submit picture proof via email.

Based on observation and interview, the licensee did not comply with the section cited above in wall heater barricade located in living room not securely anchored to wall which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to anchor wall heater barricade to wall and submit picture proof via email.

Based on record review LPA observed Maria Vizcarra completed Child and Infant CPR through Emergency Response Training EMS Safety CPR & First Aid on 12/6/21. LPA did not observe EMSA stickers. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will check with the instructor to see if the can provide EMSA stickers and go to EMSA.CA.GOV website.

Based on record review LPA did not observed staff #1, #2 and #3 did not have proof of influenza, MMR and Tdap the licensee did not comply which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will keep files organized. The files will be kept in the folder in their own dividers along with the childrens files.

Based on record review LPA did not observe a disaster drill log. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licesee stated that she usually conducts the drill with her representative from the Sycamores office and she logs them. Licensee stated that going forward the licensee will document the drills and keep it in the folder with the childrens files.

Resolution: Licensee stated that she and staff will take the training and set a reminder in the calendar to renew training.

Based on observation and record review, LPA did not observe Aaaron Vizcarra associated to the facility. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will register for Guardian to help with ensuring who is associated to her facility.
