Based on LPA observation and record review of facility assoications, the licensee did not comply with the section 1596.871(c)(1)(A) in Adult 1 did not have criminal clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Adult 1 left facility at 12:50PM. Licensee will submit proof of contacting Guardian to inquire about Adult 1's clearance since status was listed as in process. Licensee provided a delcaration stating Adult 1 will not be present at facility until clearance is processed.

ased on LPAs interview and observation, Adult 1(A1) did not have a clearance associated to facility at time of inspection. Licensee states A1 has clearance with another facility. A1 states she helps with daycare and lives in the home. This is an immediate health, safety, and personal rights risk to children in care.

Resolution: Licensee states she will provide fingerprint clearance transfer paperwork or proof that A1 took Livescan fingerprinting by POC date.

Based on LPAs observation, Licensee did not have proof of purchase receipt or service tag for fire extinguisher which is a potential health, safety, and personal rights risk to children in care.

Resolution: Licensee states she will obtain proof of purchase receipt by POC date via email.

Based on LPAs record review and interview, Licensee did not have proof of Child Care Provider (AB1207) specific training at time of inspection. This is a potential health, safety, and personal rights risk to children in care.

Resolution: Licensee states she took training and provided proof of training at the end of visit, clearing deficiency on same date.

Based on LPAs record review at 3:19 PM, Licensee states they do not have records or medical exemption from doctor at this time which is a potential health, safety, and personal rights risk to children in care.

Resolution: Licensee states she will provide proof of immunization and/or exemption by POC date via email.
