Based on observation, the licensee did not comply with the section cited above in LPA observed he backyard water heater door with multiple areas of chipping paint which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will esnure the facility is free from defects and conditions which might endanger a child. Licensee will ensure to remove all paint chippings. POC will cleared by visit.

Based on observation, the licensee did not comply with ensuring fire extinguisher was purchased or serviced yearly which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with ensuring Mandated Certificate was current and available for review which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in that there is active consruction in the kitchen resulting in backyard, two bedrooms used for care are temporarily inaccessible which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the daycare is provided on the living room and outdoor play is provided in front yard, children are supervised at all times. The licensee will complete a plan of supervision while construcion is being done.

Based on observation, the licensee did not comply with the section cited above by having toys and stuffed animals in the play yard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed all items from the play yards during the inspection.

Based on record review, the licensee did not comply with the section cited above by not having the mandated reporter training certificate for review at the time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof to LPA of completed mandadted reporter training via email

Based on observation, the licensee did not comply with the section cited above by having two baby walkers in the day care room which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immidiately removed the walkers and placed them in the off limits garage during inspection.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 children did not have immunization records available at the time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of childrens immunizations and completed file of all children enrolled at the facility to LPA via email.

Based on record review, the licensee did not comply with the section cited above by not having proof of immunizations at the time of inspection which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of immunizations to LPA via email.

Based on observation, the licensee did not comply with the section cited above by having a door leaning against the detached garage that had a protruding object coming through the door, paint thinner under plastic chair and feces on a child’s scooter in the accessibke area of the backyard which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of removal of door leaning against the garage and removal of feces on the childs scooter via email. Licensee removed paint thinner and painting materials during the inspection and placed them in the off limits garage.

Based on observation, the licensee did not comply with the section cited above by having tricycles with spider webs on them, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee will provide LPA proof of removal of sider webs via email.

Based on observation, the licensee did not comply with the section cited above by not having a fire extinguisher serviced or purchased in the past year which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of purchase or service of fire extinguisher to LPA via email.
