Based on record review, the licensee did not comply with the section cited above in that S2 was missing immunization record which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain S2's immunization record and submit to Community Care Licensing (CCL) by Plan of Correction (POC) due date.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above. The following was found in the home during LPAs inspection chemicals and medication were found in the kitchen, on-limits bathroom and hallway closet. Two razors were found in the bathroom and dog feces were found in the backyard, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee picked up medication, chemicals and razors during the inspection and placed them in an off-limit area. Licensee agrees to remove dog feces by 06/17/2022 and send proof to licensing.
