Based on record review, the licensee did not comply with the section cited above in that C1 is missing the Individual Infant Sleeping Plan [LIC 9227 (3/20)] which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the Individual Infant Sleeping Plan [LIC 9227 (3/20)] for C1 signed by the authorized representative(s) and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.
Based on record reviewed, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review a current Mandated Reporter certificate for Licensee.
Resolution: Licensee understands all staff working with children must have a current Mandated Reporter certificate on file and available during inspection. Licensee understands Mandated Reporter must be completed every two years. Licensee agrees to complete training and submit the certificate of completion to the Department on or by POC due date of 01/16/2026. Training can be taken at www.Mandatedreporterca.com.
Based on the observation, the Licensee did not meet the above regulation which poses a potential Health & Safety risk to the children in care. During the facility tour the LPA observed a fire extinguisher that was not fully charged. The facility does have a fire extinguisher rated 1-A:10-B:C and no children were present. Facility has been closed for two weeks due to the holiday.
Resolution: Licensee understands a facility must have a fully charged fire extinguisher rate 2-A:10-B:C. Licensee understands when children are present without a fully charged fire extinguisher it is an immediate risk. Licensee agrees to send the Department proof of having a fully charged 2-A:10-B:C fire extinguisher on or by POC due date of 1/16/2026.
Based on record reviewed, the Licensee did not meet the above regulation which poses a potential Personal Rights risk to the children in care. During the children’s file review the LPA was unable to review an LIC 995 Parents Rights Receipt for C2 and C3.
Resolution: Licensee understands all children enrolled must have an LIC 995 signed by their Authorized Representative and a copy must be in their file and available for review during the inspection. The Licensee agrees to obtain an LIC 995 for C2 and C3, send a copy to the Department on or by POC due date of 01/16/2026.
Based on records reviewed, the Licensee did not meet the above regulation which poses a potential Health & Safety risk to the children in care. During the children’s file review the LPA was unable to review an LIC 627 Consent For Emergency Medical Treatment for C2 and C3.
Resolution: Licensee understands all children enrolled must have an LIC 627 signed by their Authorized Representative and a copy must be in their file and available for review during the inspection. The Licensee agrees to obtain an LIC 627 for C2 and C3, send a copy to the Department on or by POC due date of 01/16/2026.
Based on record review, the licensee did not comply with the section cited above. Last disaster drill was conducted: 05/05/24. Licensee was reminded that disaster drills should be conducted every 6 months. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to conduct an emergency disaster drills with the daycare children and submit proof of date completed, on an emergency disaster drill log.
Based on record review, the licensee did not comply with the section cited above. Licensee did not provide proof of MMR, Tdap and/or Flu shot/declination of flu shot for herself. Licensee was reminded that anyone working with the daycare children must have all immunizations on file (measles (MMR), pertussis (Tdap) and Flu shot (or written declination of flu shot). This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of immunizations (MMR, Tdap, Flu shot/written declination) to the department no later than POC due date.
Based on record review, the licensee did not comply with the section cited above. Licensees CPR/FA expired: 10/2024. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of updated CPR/First Aid card for herself and anyone assisting in providing and being left alone to care for children, no later than the POC due date.
Per licensees own admission, licensee does not document 15 minute checks on infants. Licensee is reminded infants for licensing are any children under 24 months of age. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to immediately begin implementing 15 minute checks and logging them. Proof of an infant sleep log is due to the department no later than the POC due date.
Based on record review, the licensee did not comply with the section cited above in [3] out of [4[ Mandated reporting certificate (S1,S2,S3) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit completed copies of Mandated reporting certificates for Licensee, Daughter and Employee by above dated to LPA Steven Montoya email address: Steven.Montoya@dss.ca.gov.
