Based on observation, the licensee did not comply with the section cited above in that there was one crib that had a sheet that did not fit tightly on the mattress, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit proof (pictures) of the new sheet on the mattress. These pictures will include one picture of the top of the mattress and one picture of the bottom of the mattress, while the new sheet is on the mattress. Facility will submit the above items to the Department by the POC due date.
Based on record review and interview, the licensee did not comply with the section cited above in that one staff member (Staff 1 [S1]) did not renew their mandated reporter training every two years, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit proof (pictures) of a completed child care provider mandated reporter training that satisfies Assembly Bill 1207 by the POC due date.
Based on record review and licensees own admission the licensee did not comply with the section cited above. There was no LIC9227 on file for the 1 infant in care who was under 12 months of age. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to get an LIC9227 filled out and submitted to the department no later than the POC due date.
