Based on record review, the licensee did not comply with the section cited above in that C1 did not have an Individual Infant Sleeping Plan (LIC9227) on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1's authorized representative(s) complete the Individual Infant Sleeping Plan (LIC9227) form and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPA observed two knives on a drying rack in the kitchen that is an area that is accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed knives and placed them in an inaccessible area of the kitchen. Licensee agrees to submit a written statement of understanding and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date. Additionally, Licensee will have authorized representative(s) complete the LIC9224 form and place in children's file for one year of today's date.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee stated she did not have the 15 minute sleep checks documented for C1 due to child returning to daycare on 02/16/2026, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document the 15 minute sleep checks for C1 and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that C3 did not have the Notification of Parent's Rights form on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C3's authorized representative(s) complete the Notification of Parent's Rights LIC 995A form and provide proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that three out of four children's records were missing the LIC 995A which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have authorized representatives complete the LIC 995A for C1-C3 and submit proof to Community Care Licensing by Plan of Correction (POC) date above.

Based on record review, the licensee did not comply with the section cited above in that three out of four children's records were missing the LIC 9150 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have authorized representatives complete the LIC 9150 for C1-C3 and submit proof to Community Care Licensing by Plan of Correction (POC) date above.

Based on interview and record review, the licensee did not comply with the section cited above in that C4 was missing the fifteen minute sleep check log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete fifteen minute sleep check log for C4 and submit proof to Community Care Licensing by Plan of Correction (POC) date above.

Based on observation, the licensee did not comply with the section cited above in that poisons/toxins were not key locked and were accessible to children in day care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to move poisons/toxins to a key locked area and submit proof to Community Care Licensing by Plan of Correction (POC) date above.

Based on record review, the licensee did not comply with the section cited above in that S2 did not have a mandated reporter child care providers certificate on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S2 complete the mandated reporter training and submit proof to Community Care Licensing by Plan of Correction (POC) date above.

Based on record review, the licensee did not comply with the section cited above in that one out of four day care children's immunization records were not updated which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update C4's immunization record and submit proof to Community Care Licensing by Plan of Correction (POC) date above.

Based on record review, the licensee did not comply with the section cited above in that infant in care did not have a 15 minute sleep log, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA went over safe sleep regulations and provided a copy. Licensee agrees to conduct and document 15 minute checks on C1 and submit proof of log to CCL by 7/28/23.
