Based on record review and interview, the licensee did not comply with the section cited above due to their mandated reporter certificate expiring August 2025 and their assistant having not completed the training. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation, record review, and interview, the licensee did not comply with the section cited above due to their pool alarm not being in full working condition during the visit and not being compliant with ASTM standards. This poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Licensee stated they will purchase the correct ASTM compliant alarm, provide LPA with documentation proving it is ASTM complaint, and also provide copies of a week of morning safety check logs by the POC due date. Based on interview and record review, the licensee did not comply with the section cited above due to not being able to produce proof of influenza, pertussis, or measles immunization for their assistant, Staff 1. This poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Licensee will provide LPA Gallo copies of influenza, pertussis, and measle immunizations for Staff 1 by the POC due date. Documents can be provided by email at matthew.gallo@dss.ca.gov or by text at 916-208-3734.

Based on record review, the licensee did not comply with the section cited above in 9 out of 12 children's files reviewed by LPA, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above by being unable to produce 15 minute sleep checks for infants in care. This poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Licensee will send LPA Gallo proof of 15 minute sleep logs for the period of 6/28/2023 to 7/7/2023 to ensure compliance with regulation

Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed chemicals unlocked under the sink in the kitchen.

Resolution: Licensee moved all cleaning compounds to the garage during the inspection.

Based on record review, the licensee did not comply with the section cited above in 4 out of 5 files reviewed which poses/posed a potential health, safety or personal rights risk to persons in care. Three files were missing the LIC282, two files were missing LIC627, one file was missing the LIC700, and three files were missing the PM286.

Resolution: LIcensee will email LPA Sutter with proof of completed child files.

Resolution: Licensee stated that she is currently enrolled for a CPR/First Aid course on August 2, 2022. Licensee will submit proof of completed course by the POC date listed above.
