Based on observation, the licensee did not comply with the section cited above in that C1 was sleeping with in the play yard with blankets which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed blankets while LPA was present. Licensee will submit written statement confirming they have reviewed and understand all safe sleep regulations. Licensee will submit statment to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/28/2025.
Based on observation, the licensee did not comply with the section cited above in that C1 was sleeping clip attached to pacifier which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit written statement confirming they have reviewed and understand all safe sleep regulations. Licensee will submit statment to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/28/2025.
Based on record review, the licensee did not comply with the section cited above in that Licensee's and 2 staff members mandated reporter training expired 07/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send updated mandated reporter training certificate for Licensee and 2 staff members to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/28/2025.
Based on record review, the licensee did not comply with the section cited above in that C5 and C8 did not have signed LIC627 form (Consent for Emergency Medical Treatment) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will obtain signed LIC627 for C5, C8 and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/28/2025.
Based on observation and record review, the licensee did not comply with the section cited above in that Licensee did not have an assistant present and was alone with 8 children (3 infants, 5 toddlers) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with staffing ratio and capacity. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 08/28/2025
Based on observation, interview, record review, the licensee did not comply with the section cited above that Adult #1 lives in the home and has not been fingerprinted, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Assistant agrees that Adult #1 will need to be fingerprinted and cleared before living in the home. Assistant agrees to communicate with licensee regarding this plan of correction and will send proof that A1 has been fingerprinted to the Department by 08/30/2023.
Based on interview, record review, the licensee did not comply with the section cited above in that Assistant 1 has no documentation of required immunizations (pertussis and measles, mumps, rubella) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Assistant agress to communicate with licensee regarding this plan of correction and will submit proof of immunizations to the Department by due date 09/15/2023.
