Based on record review, the licensee did not comply with the section cited above in that Licensee's mandated reporter training expired 12/06/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send updated mandated reporter training certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/28/2026.
Based on observation and interview, the licensee did not comply with the section cited above in that Licensee was alone with 8 children (3 infants/5 toddlers) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will provide a written statement attesting she understands and must comply with the capacity of a small family child care home when there is no assistant present at her home which is licensed as a large.
Based on record review, the licensee did not comply with the section cited above in 5 of 9 children files reviewed did not have LIC 9224 on file for a type a violation issued last year,which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will submit signed LIC9224 to the Department.
Based on record, the licensee did not comply with the section cited above in that assisant (S2) had expired mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit S2 updated mandated reporter training certificate.
Based on interview and record review, the licensee did not comply with the section cited above in Adult #1 lives in the home and does not have a background check clearance or exemption which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee states will ask Adult #1 (A1) to move out of the home today after day care hours. Licensee will also provide a written statement attesting A1 no longer lives in the home and will provide documentation that A1 no longer lives in the home.

Resolution: Licensee states will bring condition of fence to good repair and will provide proof of the Department by 01/29/2024.
