Resolution: Licensee stated they will submit photo update to LPA confirming firearm and ammunition are stored and locked separately. Licensee will submit proof by 12/12/2024.
Based on interview and record review, the licensee did not comply with the section cited above in that S1 and S2 did not have updated mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit S1's and S2's updated mandated reporter training. Licensee will submit to LPA via email by 12/17/2024.
