Based on record review, the licensee did not comply with the section cited above in that Licensee's Mandated Reporter expired 08/04/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send updated mandated reporter training certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/12/2025.
Based on record review, the licensee did not comply with the section cited above in that Licensee was not able to provide complete sleep logs for C1 which poses a potential health, safety or personal rights risk to persons in care. Licensee did not 15 minute sleep logs for C1 from January 2025 thru July 2025.
Resolution: Licensee stated they will locate the missing 15 minute sleep logs and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/12/2025. If Licensee is not able to locate sleep logs, Licensee will send statement confirming they understand the importance of maintaining the 15 minute sleep log documentation.
