Based on record review, the licensee did not comply with the section cited above in that C2 did not have LIC627 form (Consent for Emergency Medical Treatment) on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain signed LIC627 for C2. Licensee will send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/08/2025.

Based on record review, the licensee did not comply with the section cited above in that the last emergency drill was conducted on 09/02/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will conduct an emergency drill with children in care and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/08/2025.

Based on observation, the licensee did not comply with the section cited above in that there were two infants present but only one play yard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will review infant safe sleep regulations and send email confirming they have read and understand regulations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/08/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have immunization record for C1 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send copy of C1’s complete immunization record to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/08/2025.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have LIC700 for C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain signed LIC700 (Identification and Emergency Information Child Care Centers/Family Child Care Homes) for C2. Licensee will send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/08/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have 15 minute sleep logs for C1 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send a weeks’ worth of 15 minute sleep documentation of all infants in care to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/08/2025
