Based on observation, the licensee did not comply with the section cited above in that infant in care, C1, was sleeping in play pen with blank which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed blanket from play pen while LPA was onsite, Licensee will read and review safe sleep regulations. Licensee will send email to LPA confirming they have read and understand safe sleep regulations by 11/13/2024.
Based on inteview and record review, the licensee did not comply with the section cited above in that assistant, S1, did not have required record of immunizations ( Measles, Pertussis and Influenza) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA proof of assistants immunization record by 11/13/2024.
Based on interview and record review, the licensee did not comply with the section cited above in that there was no sleep log documentation for the one infant in care, C1, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA a weeks worth of sleep log documentation for the infant in care and submit to LPA by 11/13/2024.
