Based on interivew and record review, the licensee did not comply with the section cited above in that Licensee's and S2's mandated reporter training expired 06/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send updated mandated reporter training certificates for themselves and S2 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/29/2025
Based on interview and record review, the licensee did not comply with the section cited above in Licensee was not able to provide documentation of 15 minute sleep logs for C6 which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send a weeks’ worth of 15 minute sleep documentation of all infants in care to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/29/2025.
Based on observation, the licensee did not comply with the section cited above in Adult son's bedroom and teenage daughters bedroom were not locked at time of inspection. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will get locks for the doors and send LPA Flores photos via email by 10/22/23.
