Based on record review, the licensee did not comply with the section cited above in that the last emergency drill was completed on November 2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct fire drill with children in care and submit proof to LPA by 04/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that Assistant 1 and Assitant 2 and licensee do not have a current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will submit completion of mandated reporter training for self and current assistant(s) by due date 04/12/24.

Based on interview and record review, the licensee did not comply with the section cited above in Assistant 1 who provides transportation to children in care does not have current CPR and First aid training on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide documentation of completed CPR and First aid training for Assistant 1 by due date 04/27/2024.

Based on interview and record review, the licensee did not comply with the section cited above in that three files of six reviewed were not available for review and missing LIC 700 and LIC 627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide proof/documentation of forms needed on children records for C1, C2, C3, by due date 04/12/2024.

Based on interview and record review, the licensee did not comply with the section cited above in thee children (C1, C2, C3) of six children files reviewed, did not have files available for review at the time of inspection and missing LIC 995a, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will submit missing forms (LIC995a) for C1, C2, C3 by due date 04/12/2024.

Based on record review, the licensee did not comply with the section cited above in Licensee spouse/assistant did not have proof of completion of the mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to have assistant/spouse completed the required mandated reporter training and submit copy of certificate.

Based on record review, the licensee did not comply with the section cited above in that assistant/spouse did not have proof of pertussis and measles on file posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of Pertussis and Measles for assistant/spouse-Juan Garcia, submit proof to the department by 3/23/2022.
